Community Network Director

Reporting to the Regional Director, the Community Network Director is responsible for managing, developing, and growing the NCServes Fayetteville Network. The Community Network Director does this by facilitating connections between existing network partners, out of network resources, community stakeholders and Veterans utilizing a collective impact, community of practice model. The Director is responsible for maintaining and strengthening existing relationships, identifying, and recruiting new resources, growing community awareness of NCSERVES, and providing leadership and technical assistance to local Community of Practice. The Director plans, tracks and implements community engagement activities such as training, education, outreach, provider fairs and the annual NCServes In-progress Review. Monitors, interprets, and reports Network performance indicators. Provides leadership and guidance to the Community Partner Liaisons, and Care Coordination staff. In conjunction with Regional Network Director manages care coordination of complex Veterans to identify and connect them to available community resources. Manages and supervises the BSW/MSW intern program. Serves as a member of the Fayetteville Community Veterans Engagement Board and Triad Community Blueprint. Serves as the NCSERVES representative on the VBH leadership team, contributes to strategic planning and implementation.

Reporting to the Community Network Director, the Community Care Coordinator is responsible for functioning as a subject matter expert on military and Veteran families specializing in specific service domains (i.e., Employment, Housing, Financial Assistance, etc.) and/or a particular geography (i.e., rural counties, urban centers, etc.-resources). Identifies and establishes new relationships with providers to improve service delivery within assigned geographic area. Enrolls providers into utilization of Unite Us. Identifies and prioritizes incoming Veteran referrals, does outreach to identified Veterans and connects to resources. Seeks out new resources to fill service delivery gaps as needed. Builds community care teams using Unite Us technology. Facilitates collaboration between direct services providers and assist Veterans, Military Members, and their families to connect to resources for which they are eligible or may benefit from as needed. Manages data and technology related to assistance requests, referrals, referrals needing action, and open cases to promote participation in NCServes Network.

The Community Care Coordinator will assist in our growth into South Carolina. This person will identify and establish new relationships with providers to improve service delivery within York, Lancaster, Chester, and Chesterfield Counties.
